The following forms are required for all BHS drama participants:

Alachua County Volunteer Application - required to volunteer with the Theater Department in any capacity

Overnight field trip permission slip

Health insurance card copy (front and back) must be attached

Request to transport students in a private vehicle

Driver's license must be attached

Vehicle registration copy must be attached

Driver forms will be rejected if:

Any information is expired, or WILL EXPIRE by the end of our season (late May)
